The purpose of The Patrick Donnelly Foundation, Inc is to honor the life of Patrick E. Donnelly of
Sturbridge, Massachusetts and his spirited passion for learning and drive to be educated. To award
scholarships to, selected qualified applicants who demonstrate themselves to be deserving college
bound high school seniors, so as to further their higher education. The corporation's vision and
dedicated purpose is to continue Patrick Donnelly's memory for increased education and enlightenment
through special fund-raising events, donations and scholarship awards.

The Patrick Donnelly Memorial Scholarship, in the amount of $5000.00 will be awarded to a Tantasqua
Regional High School Senior enrolled in Westfield State University in the school year starting fall 2013.

Eligibility for scholarship consideration:
Current High School senior at Tantasqua Regional High School in good standing
Acceptance to Westfield State University, as an incoming freshman (Fall 2013)

Required documents and process:
Completed application form
One to three letters giving examples of 'Paying it Forward' * from people you have helped.

All applicants will be contacted for an interview shortly after the application deadline.

Upon completion of interview, finalists will be chosen and contacted to please continue
Patrick's unfinished story.

* Paying it Forward - These are letters of gratitude from any individual that states that you exemplified a
spontaneous act of kindness that therefore had a positive impact on that individual's life, day or moment.

The scholarship will be granted and paid after the first freshman semester is successfully completed. (Fall 2013) Official
transcripts must be submitted to the foundation for verification.
